In questo tipo di intervento, un generatore di impulsi (chiamato pacemaker) viene impiantato nel torace per regolare il ritmo del cuore.
Preparazione
La preparazione all’intervento prevede che il paziente venga posizionato secondo quanto richiesto dal procedimento , mente il sito di inserimento sul torace viene accuratamente anestetizzato.
Al paziente viene somministrato un sedativo, che lo farà sentire rilassato e assonnato, mentre diversi monitor sono collegati al torace e al corpo del paziente.
Il team di assistenza chirurgica osserverà da vicino i monitor con i dati per controllare il ritmo cardiaco del paziente e la pressione sanguigna durante il procedimento.
Impianto Guida
Il medico crea una o più piccole incisioni per accedere a una grande vena nel torace per inserirvi un elettrocatetere guidandolo con attenta precisione nel cuore grazie anche all’aiuto di un fluoroscopio (una telecamera che crea una radiografia in movimento in tempo reale).
Solitamente l’elettrocatetere è posizionato nel ventricolo destro del cuore. Per alcuni pazienti, possono essere impiantate due derivazioni (una nell’atrio destro e una nel ventricolo destro). Questo tipo di pacemaker è chiamato pacemaker a due camere.
Testare l’Impianto guida
Dopo che l’elettrocatetere è in posizione, questo viene temporaneamente collegato a un dispositivo che invia una serie di impulsi al cuore. Questo test è chiamato “stimolazione” e consente al medico di assicurarsi che l’elettrocatetere funzioni correttamente.
Impianto del pacemaker
Quando il test è stato eseguito , il dispositivo di test viene scollegato e l’elettrocatetere viene collegato al pacemaker. Il medico crea una piccola tasca sotto la pelle del torace e fa scivolare il pacemaker in questa tasca. Una volta attivato, il pacemaker genererà impulsi elettrici e invierà questi segnali attraverso gli elettrocateteri e nel cuore.
Fine della procedura e trattamento successivo
Quando l’impianto è completato, il medico chiude le incisioni che aveva praticato, regolando poi le impostazioni del pacemaker con un dispositivo di programmazione esterno per mettere a punto il ritmo cardiaco. Il paziente rimarrà in ospedale per tutta la notte in modo che il cuore possa essere prudentemente monitorato da vicino.

Preparation
In preparation for the procedure, the patient ispositioned and the insertion site on the chest isanesthetized. The patient is given a sedative. Implanting the Lead
The physician creates one or more small incisionsto access a large vein in the chest. The physicianinserts a lead into this vein and carefully guides itinto the heart with the help of a fluoroscope (acamera that creates a real-time moving x-ray).Typically, the lead is placed in the heart’s rightventricle. For some patients, two leads may beimplanted (one in the right atrium and one in theright ventricle). This type of pacemaker is called adual-chamber pacemaker.
Testing the Lead
After the lead is in position, the lead is temporarilyconnected to a device that sends a series ofimpulses to the heart. This test is called “pacing.” Itallows the physician to ensure the lead is workingproperly.
Implanting the Pacemaker
When the test is complete, the testing device isdisconnected and the lead is attached to thepacemaker. The physician creates a small pocketbeneath the skin of the chest and slips thepacemaker into this pocket. Once the pacemaker isactivated, it will generate electrical impulses andsend these signals through the leads and into theheart.
End of Procedure and Aftercare
When the implantation is complete, the physiciancloses the incisions. The physician may adjust thesettings of the pacemaker with an externalprogramming device to fine-tune the heart’s rhythm.The patient will remain in the hospital overnight sothe heart can be closely monitored.
